Indian investigators' preliminary report on the crash of an Air India flight that killed 260 people has put the focus squarely on the pilots and the aircraft's fuel switches.

A preliminary report released last week included detail about the fuel, saying that the fuel to the plane's engines appeared to have been shut off.

On June 12, the Air India Boeing 787-8, operating a flight from Gujarat' Ahmedabad to London Gatwick, crashed into a building shortly after takeoff.
